Health Coaching and Remote Patient Monitoring
For more than a decade, the Care Collaborative’s team of health coaches has helped rural Kansans with complex medical conditions, providing more than 150,000 chronic care management and remote patient monitoring visits. The Care Collaborative also assists rural providers establish and maintain their own CCM and RPM programs. Since 2022, the Care Collaborative has partnered with Fort Hays Northwest Tech to provide customized health coach training.
The Care Collaborative will provide remote patient monitoring services in support of two RHT Plan programs: Accountable Food Is Medicine/Community Health Worker Deployment and Integrated Care for Dual Eligible Beneficiaries.
Additionally, the Care Collaborative will launch an asynchronous Diabetes Prevention Program for rural Kansas residents. In addition to delivering on-demand customized course content, Care Collaborative health coaches will meet virtually with program participants to address individual concerns and answer questions.
