In-Home Certified Medical Assistant Pilot Program

The Care Collaborative will partner with five rural hospitals to pilot in-home services for at-risk patients.  Specially trained certified medical assistants (CMAs) will make home visits for patients not eligible for home health or similar benefits referred to the program by local providers. Such patients may include individuals recently discharged from the hospital, those identified as fall risks, and patients needing support with upstream drivers of health.  The CMAs will provide services consistent with protocols developed by the program’s Medical Director and their work will be supervised by the Medical Director.

The Care Collaborative is now finalizing arrangements with the participating hospitals. The individuals who will serve as CMAs will begin training this fall through a virtual program sponsored by Fort Hays Tech Northwest. Over the course of the pilot program, the Care Collaborative will evaluate the impact of in-home services on outcomes and total cost of care to support wide-scale adoption and long-term sustainability.
