Transforming Rural Healthcare, Together
Since 2014, the University of Kansas Health System Care Collaborative has partnered with rural Kansas providers to expand access, improve quality, and achieve financial sustainability. Now, the Care Collaborative has a leadership role in implementing Kansas’ Rural Health Transformation Plan. Learn more about the programs supported by the Care Collaborative.

The federal Rural Health Transformation Program is a five-year $50 billion investment to support each state’s implementation of its transformation plan. For Year 1, Kansas received $221.89 million, the sixth largest award under the Program. Future funding will depend on the State’s progress in implementing its plan.

Kansas’ Rural Health Transformation Plan is supported by the Centers for Medicare & Medicaid Services (CMS) of the U.S. Department of Health and Human Services (HHS) as part of a financial assistance award totaling $221.89 million in Budget Period 1 with 100 percent funded by CMS/HHS.
